What is the mechanism of action of Avalide (Irbesartan + Hydrochlorothiazide)?
Avalide and its generic versions are made of two synergic-acting components – Irbesartan and Hydrochlorothiazide. Irbesartan is an angiotensin II receptor antagonist that prevents the blood vessels narrowing caused by the natural substance angiotensin II. The drug promotes the decrease of the peripheral resistance of the blood vessels, causes an increase in glomerular filtration rate and renal blood flow, as well as affects plasma renin activity.
Other blood pressure medications that are included in the class of angiotensin-converting enzyme inhibitors affect the metabolism of bradykinin (a natural peptide that promotes blood vessels extension), but Generic Avalide (Irbesartan and Hydrochlorothiazide) does not affect it or other peptides. This is one of the reasons why the medication is better tolerated than the other group of the hypotensive drugs.
The other component of the medication, Hydrochlorothiazide, is a thiazide diuretic that prevents accumulation of salts in the body which in its turn can result in swelling.
How to use Generic Avalide?

The pills are taken orally without chewing and are washed down with sufficient amount of water. The regular dosage for patients without any other chronic conditions is 150 mg a day taken in one intake regardless of meal time. This dosage is optimal for stable blood pressure reduction for 24 hours. However, in individual cases, especially in patients over 75 years of age or those who are getting hemodialysis, the dosage adjustment may be required. For these groups of patients the recommended initial dosage is 75 mg a day.
In people with known arterial hypertension or type 2 diabetes, the treatment should be started with the dose of 150 mg per day and gradually increased to 300 mg a day. The dosage of 300 mg a day is the preferred maintenance dose for the treatment of nephropathy.
In patients with water-electrolyte imbalance, before the start of the treatment, a restoration of the volume of circulating blood or elimination of hyponatraemia is required.
People with impaired kidney function do not require the dosage regimen adjustment. In visible violations of normal liver functions of mild or moderate severity, the dosage also doesn’t require adjustments.
For patients on hemodialysis during the treatment, the initial dose in most cases is 75 mg per day.
The safety as well as effectiveness of the drug in patients under the age of 18 years have not been studied sufficiently yet.
